Residential Supports in the Group Home Setting

At Acorn, Residential Support Services (RSS) are provided in a three-person residence by a service provider who is present in the residence and awake whenever an individual is present in the residence.  Direct Care Staff are assigned on a daily shift schedule that includes at least one complete change of service providers each day.

RSS includes the following service elements:

  1. direct personal assistance with activities of daily living (grooming, eating, bathing, dressing, and personal hygiene);

  2. assistance with meal planning and preparation;

  3. securing and providing transportation;

  4. assistance with housekeeping;

  5. assistance with ambulation and mobility;

  6. reinforcement of professional therapy activities;

  7. assistance with medications and the performance of tasks delegated by an RN;

  8. supervision of individuals' safety and security;

  9. facilitating inclusion in community activities, use of natural supports, social interaction, participation in leisure activities, and development of socially valued behaviors; and

  10. habilitation, exclusive of day habilitation. 

Supervised Living Services (SL) are provided in a three-person residence by a service provider who provides services and supports as needed by the individuals residing in the residence and is present in the residence and able to respond to the needs of the individuals during normal sleeping hours.  SL services include all the same service elements as RSS.
